Periodontology class was taken by Dr. John Kazimm at AHEAD Academy, New Delhi. The topic of discussion was ‘Furcation’. It was attended by students preparing for NBDE, NDEB, USA and MDS. The etiologies of furcation involvement may include anatomic factors, extension of inflammatory periodontal disease, trauma from occlusion, pulpo-periodontal disease and root fracture involving furcations. Successful treatment to retain furcation-involved teeth remains one of the most difficult treatments in overall periodontal therapy. Using the treatment protocol of scaling & root planning as a single measure or combined with surgical procedures and proper maintenance care, in multirooted teeth with furcation involvement, a high percentage of tooth mortality has been reported.

Gummy Smile Teartment at AHEAD Academy, Mohali

A patient at AHEAD Academy, Mohali complained of gummy smile. Crown lengthening was performed. Incisal bevel was given followed by circular incision. Bone cutting and suture was given and coe pack was placed. A short clinical crown occurs as a result of, deep subgingival caries, tooth fracture, iatrogenic factors may lead to poor retention form which allow for proper tooth preparation. Surgical crown lengthening procedure is done to increase the clinical crown length without violating the biologic width.

Vestibuloplasty at AHEAD Academy, Mohali Centre...

Vestibuloplasty was performed by student at AHEAD Academy, Mohali centre, under the guidance of Dr. John Kazimm.  Following complete loss of teeth, the alveolar bone undergoes various degrees of resorption, particularly in response to tissue-supported conventional dentures. This results in loss of vestibule and decreased surface area for denture support. In the previous class at AHEAD Academy, various vestibuloplasty procedures were described for restoring the vestibular depth and increasing denture surface area. Additionally, the application of vestibuloplasty in management of patients with bone augmentation and implants who commonly present with decreased vestibular depth, inadequate zone of keratinized gingiva, and thin soft tissue biotype was discussed. A step-by-step surgical technique was explained that will increase vestibular depth, zone of attached gingiva, and soft tissue thickness for enhanced aesthetics and function of dental implants.

Endodontics Demonstration at AHEAD Academy...

A demonstration in endodontics was held as part of clinical certificate course in endodontics. The class was taken by Dr. Nitin Singh. The topic of discussion was Biomechanical preparation. Modern techniques for preparing the root canal involve a crown-down approach to more efficiently remove infected debris and to improve access for irrigants. The balanced-force technique, with a 60° clockwise turn followed by a balanced anti-clockwise cutting motion, is accepted as the most efficient method of file manipulation. Copious irrigation with an appropriate antiseptic material is essential to clean the root canal system following shaping of the main canals. The smear layer should be removed with an EDTA solution before placing an intervisit dressing, or carrying out obturation.

Splinting Of Fractured Tooth at AHEAD Academy...

Splinting of anterior loose teeth was done for a lady who met with an accident three months back.. Although the techniques require a high degree of clinical skill and diagnostic expertise, they save a significant number of teeth. Gum disease is the main cause of loose teeth in adults and of tooth loss. Gum disease, if left untreated will result in the loss of the surrounding tissues that hold teeth in place. Anterior dental trauma is a common injury pattern of the dentoalveolar system in which appropriate first aid is important. When children and adolescents suffer an anterior dental trauma, the challenge is to preserve the tooth in this esthetically important area and minimize subsequent damage. Lack of treatment or improper treatment can lead to a lifelong need for dental care.

Impaction Case at AHEAD Academy...

Dr. Arpit performed an impaction case himself at AHEAD Academy. He was happy to get such an opportunity to do such cases independently. Tooth impaction is defined as any tooth that fails to erupt into a normal functional position and remains unerupted in the jaw beyond the time when it should normally erupt.  Such cases are regularly performed at our centres under the guidance of expert teachers. Soft tissue impaction occurs when the crown of the tooth has penetrated through the bone, but the gum is still covering part of the tooth. When the tooth has partially erupted, but a part of the tooth remains submerged in the jawbone, this is considered partial bony impaction. Complete bony impaction occurs when the tooth is entirely encased by jawbone. Impacted wisdom teeth do not always show symptoms, meaning you could have impacted teeth and not even realize it. If symptoms do arise, it is usually the result of the gum on top of the tooth becoming infected or swollen. Symptoms may include pain, swollen and bleeding gums, swelling around the jaw, bad breath, headache or jaw ache, and an unpleasant taste when eating. Some people experience stiffness of the jaw or swollen lymph nodes in the neck.

Clinical Endodontics Class at AHEAD Academy...

Today Clinical class in endodontics was conducted at Ahead academy. The students were taught about the technique of obturation. The practical aspects of clinical and radiographic features of obturation were discussed the tug back and working length and their relationship was taught. The use of paper points to dry all canals was emphasized. The COLD LATERAL CONDENSATION technique was taught to students. A video was shown to students where they were shown the use of a gutta percha cutter.

A case of Depigmentation was carried out at AHEAD Academy, New Delhi. A 24 year old male came to the clinic with discoloured blackish gums. The procedure was carried out by our new batch of students at AHEAD Academy, New Delhi. Gingival pigmentation is a major concern for a large number of patients visiting our clinic. The patients with excessive gingival display and pigmentation are more concerned aesthetically. Most pigmentation is caused by five primary pigments out of which melanin shows the maximum incidence rate. Melanin hyper pigmentation usually does not present as a medical problem, but patients may complain about their anaesthetic black gums. The gingiva is the most frequently pigmented intraoral tissue, with the highest rate observed in the area of the incisors.

Molecular Biology Class at AHEAD Academy

Online class was taken today by Dr. Seebu Dhingra. The topic of discussion was ‘Molecular Biology’. Students preparing for Canada and ADC exams attended the class. Questions asked in these exams were discussed, followed by a doubt session. Molecular biology is a branch of science concerning biological activity at the molecular level. The field of molecular biology overlaps with biology and chemistry and in particular, genetics and biochemistry. A key area of molecular biology concerns understanding how various cellular systems interact in terms of the way DNA, RNA and protein synthesis function. Genetics is concerned with the effects of genes on living organisms, which are often examined through “knock-out” studies, where animal models are designed so that they lack one or more genes compared to a “wild type” or regular phenotype. Molecular biology looks at the molecular mechanisms behind processes such as replication, transcription, translation and cell function. One way to describe the basis of molecular biology is to say it concerns understanding how genes are transcribed into RNA and how RNA is then translated into protein. However, this simplified picture is currently be reconsidered and revised due to new discoveries concerning the roles of RNA.

Endodontics Clinical Class at AHEAD Academy

Today clinical class in endodontics was conducted by Dr. Nitin Singh. The topic of discussion was “Canal negotiation and working length determination in RCT”. The root canal anatomy of incisors, premolars and molars was emphasized. An accurate working length determination of root canal during endodontic treatment is very essential. It makes endodontic treatment easier for an operator to remove necrotic tissue and prepare canals precisely. Various methods have been used to establish correct working length. These include use of conventional or digital radiography, tactile method and moisture on paper point. All of these methods have their limitations. Radiographs are subjected to distortion and magnification. It comprises accurate location of root apices. They are technique sensitive in exposure and interpretation. They also provide a two dimensional image of a three dimensional structure which may not represent real position of apical region. Furthermore, in many cases with conventional radiography, it is difficult to establish the actual length of the canal with a two dimensional image. It becomes even more difficult to establish correct working length with radiography when root canal system is superimposed radiographically by anatomic structures. In such cases, electronic method using apex locator is very useful.

Case of fused molar at AHEAD Academy

A patient came to the clinic with pain in his upper right region of jaw. His molars were fused together. The extra tooth was removed and the patient was relieved of pain. Fused/geminated teeth may cause esthetic, spacing, periodontal, eruption and caries problems. Fusion is an abnormality of tooth development, defined as the union at the enamel, dentine, cementum or pulp level during different stages of odontogenesis of two or more separately developing contiguous tooth germs. When fusion takes place, the pulp chambers and root canals may either be joined or separated, depending on the stage of tooth development at the time of union. The process of tooth fusion involves epithelial and mesenchymal germ layers, usually resulting in irregular tooth morphology.

Flap surgery at AHEAD Academy

 Many flap surgery cases were operated at AHEAD Academy, New Delhi. These cases were performed by students of clinical course in periodontics. Flap surgery is a technique in plastic and reconstructive surgery where any type of tissue is lifted from a donor site and moved to a recipient site with an intact blood supply. This is similar to, but different from, a graft, which relies on growth of new blood vessels. The purpose of periodontal surgery is to treat deformities and tissue loss created by the disease process, eliminating “pockets” of diseased tissue in order to create and maintain periodontal health. Techniques have been developed to deal with the gingival (gum) tissues and underlying bone. The objectives of periodontal flap surgery are to:

Eliminate or reduce pockets.

Regenerate periodontal tissues and their (re)-attachment to the teeth.

Create more normal periodontal form, function, and aesthetics;

Promote an environment more conducive to good oral hygiene practices and professional maintenance care.

Suturing Techniques...

Suturing techniques were demonstrated to the new batch of students. Different techniques were practiced by the students after the demonstration. There is a need for students to be able to make a logical decision regarding which suture materials to use in a given clinical situation because the choice of wound-closure materials may make a difference in wound healing and the ultimate functional and cosmetic result obtained. Basic suturing techniques were also reviewed. Knowledge of multiple suturing techniques provides versatility, individuality, and optimal closure of surgical wounds. Techniques that must be mastered include good eversion of skin edges, avoiding suture marks, maintaining uniform tensile strength along the skin edges, and precise approximation along skin edges.

Clinical class at AHEAD Academy

Clinical class in endodontics was conducted at AHEAD Academy, New Delhi. The topic of discussion was veneers and laminates. The types of veneering were explained to the students.. The step-by-step procedure of veneering was elaborated by the staff. The new students were explained the basic chair positions while operating a patient on dental chair. Porcelain veneers (also referred to as dental laminates), are wafer-thin shells made out of dental ceramic that are bonded onto the front side of teeth. Laminate veneers are restorations which are envisioned to correct existing abnormalities, esthetic deficiencies and discolorations. Laminate veneer restorations have two different types: direct and indirect laminate veneers. Direct laminates are applied on prepared tooth surfaces with a composite resin material directly in the dental clinic. Absence of necessity for tooth preparation, low cost for patients compared with indirect techniques and other prosthetic approaches, reversibility of treatment and no need for an additional adhesive cementing system are some advantages of this technique. Intraoral polishing of direct laminate veneers is easy and any cracks or fractures on the restoration may be repaired intraorally and marginal adaptation is better than that of indirect laminate veneer restorations.

Fracture build-up at AHEAD Academy

A 23 year old female came with fractured front teeth. A build up with composite was performed for her by Dr. Parth, a student of clinical course in endodontics. Composite resins are tooth-colored materials that can actually be applied to the remaining surfaces of teeth to replace lost tooth structure in such a way as to actually make them one, blending and exactly matching the physical characteristics and color of natural teeth, and actually strengthening them in the process. Most importantly, modern composites physically adhere by actually bonding to the two elements that teeth are composed of, dentin and enamel. Major advances have resulted from the study and understanding of how the crowns of teeth actually flex or give under biting force and how restorative materials can be used to the greatest effect in the way they interact. And, best yet, composites can be used to restore teeth directly — they are applied directly to the teeth in the dental office in a single appointment. That means no temporaries and no outsourcing to dental labs for fabrication, which also means less cost and time.

Bleaching was performed for a student with yellowish colored teeth. The procedure was performed by students of clinical course at AHEAD Academy, New Delhi.

Flap surgery at AHEAD Academy

Flap surgery was performed by Dr. Anjali at AHEAD Academy. A 46 years old man came with loose and dirty teeth. Flap surgeries are independently performed regularly at AHEAD Academy, New Delhi and Mohali centre. Flap surgery is typically done under local anesthesia, sometimes accompanied by oral anti-anxiety medications; alternatively, it may be performed under intravenous conscious sedation. After anesthesia has taken effect, a small incision is made to separate the gums from the teeth. The outer gum tissue is gently folded back to give access to the roots and the supporting ligament and bone tissue. Next, the inflamed gum tissue can be removed, and the tooth roots can be cleaned; if needed, the area may also be treated with antibiotics or other medications. Bone defects can be repaired with grafting material, and proper regeneration of the periodontal ligament can be encouraged by physical (barrier membranes) and chemical (growth factors) methods. Finally, the incision is closed and the procedure is completed. Performed by an experienced hand, state-of-the-art flap surgery has an excellent track record and offers well-established benefits. It's often the treatment of choice for relieving periodontal disease and helping to maintain your oral health — and preserve your teeth.

Course Objectives:

Post and core treatment is recommended for teeth which are severely decayed or structurally damaged. After a root canal treatment, the post is positioned in the root of the tooth. It can be used to support a filling or to hold a crown. Some posts are ready made, whereas others are made in the laboratory especially to fit the tooth. If the previous restorative therapy or dental caries has resulted in substantial loss of tooth structure, the abutment teeth for fixed prosthodontic restorations require a core reconstruction or a post and core. Small dentin defects can be restored with bonded cores; more extensive dentin defects that are often accompanied by previous endodontic treatment generally require additional support for the core material. A direct procedure with a bonded post is a viable treatment method. The decision to use either light-conducting all-ceramic zirconium or titanium as a post material depends on the aesthetic requirements present.
